Team Roster Form

Please take extra care in submitting the following information as it will be used for producing various tournament items such as t-shirts, bag tags, etc.  Once created these items cannot be reproduced so be sure to take the extra time to check that all names are spelled correctly and none of your players, coaches, or managers are missing.

For readability and consistency all information should be in lower case letters with the exception of the first letter which should be capitalized (ex. "Tom"). 

Team Rosters submitted after the registration deadline are not guaranteed tournament items requiring names for their team.

   Full payment of fees must be received within ten (10) days of submitting your online registration or your entry will be cancelled. No refunds will be issued for cancellations within 30 days of the start of the tournament.
